Informed consent for ADHD assessment
Cube Clinic provides adult ADHD assessment by telehealth. The purpose of the assessment is to determine whether the client’s presentation is consistent with ADHD, whether another explanation better accounts for the difficulties, or whether ADHD is present alongside other factors.
Assessment process
The assessment includes online booking, payment, questionnaires, ADHD-specific rating scales, informant information where available, uploaded records where available, clinical interview, structured ADHD interview, psychologist review, written report, and optional feedback session. The clinical interview and optional feedback session are conducted by an appropriately trained ADHD clinician. Diagnostic conclusions are made only after clinical review and psychologist oversight, and the psychologist remains responsible for diagnosis, formulation, and report sign off.
Nature and limits of service
The assessment is not a crisis service, emergency service, prescribing service, or ongoing therapy service. The assessment may or may not result in an ADHD diagnosis.
Questionnaire and screening process
The questionnaires assess attention, concentration, executive functioning, organisation, impulsivity, emotional wellbeing, developmental history, sleep, trauma, substance use, autism traits, and other factors relevant to differential diagnosis and co-occurring conditions. This broader screening process is important because difficulties with attention, concentration, mood, sleep, stress, trauma, anxiety, substance use, or other mental health factors can sometimes look similar to ADHD or occur alongside ADHD. The assessment therefore considers the overall clinical picture rather than relying on a single questionnaire or symptom checklist alone.
Client responsibilities
The client understands that accurate and complete information is important. Incomplete information may limit the confidence of the assessment. The client understands that informant information and childhood information are helpful in ADHD assessment. If childhood informant information is not available, the clinician will discuss alternative sources of information. The client understands that questionnaires are part of the assessment but do not diagnose ADHD by themselves. The client understands that uploaded records are optional unless specifically requested, but may assist the assessment.
Telehealth requirements
The client understands that telehealth requires a private space, stable internet connection, and a device with camera and microphone.
Digital systems and AI-assisted documentation
The client understands that Cube Clinic uses secure digital systems, secure cloud storage, secure email, and secure payment systems to provide the service. The client understands that AI-assisted documentation tools may support clinical notetaking and report drafting. The purpose of AI-assisted documentation is to support accurate clinical records and allow the clinician to focus more fully on communication, the assessment, and the client rather than manual notetaking. Secure systems processes session audio into a temporary transcript and clinical summary for the purpose of creating clinical documentation and session summaries. Audio is not stored. Transcripts are temporary and are typically deleted. A clinical summary may be saved to the client record. Systems use encrypted systems and secure Australian-based infrastructure, complying with Australian Privacy Principles and being aligned with AHPRA and HIPAA standards, and as not using client data to train AI models. AI-assisted documentation supports workflow efficiency and documentation quality but does not replace clinical judgement or make clinical decisions. The psychologist remains responsible for clinical interpretation, diagnosis, report content, and final sign off. The client may decline or withdraw consent for AI-assisted notetaking at any time.
Confidentiality and safety limits
The client understands that information is confidential except where disclosure is authorised, required by law, required for safety, required for professional consultation, or necessary for service delivery. If Cube Clinic becomes concerned about a serious and immediate risk to the safety of the client or another identifiable person, information may need to be shared with appropriate support services, emergency services, or relevant professionals in accordance with legal, ethical, and professional obligations.
Reports and factual corrections
The client understands that the report is released only after psychologist sign off. The client understands that factual inaccuracies can be raised after receiving the report.
Financial and attendance terms
The client understands the payment, cancellation, rescheduling, questionnaire completion, and non-attendance terms.
Consent confirmation
By booking and proceeding with assessment, the client confirms that they have read and understood the service information, Privacy Policy, Terms of Use, and this Informed Consent.
