Terms of use

These Terms of Use apply to the Cube Clinic Adult ADHD Assessment Service. By booking an assessment, the client agrees to these terms.

Nature of Service

Cube Clinic provides adult ADHD assessment by telehealth. The service includes pre-assessment questionnaires, ADHD-specific rating scales, informant information where available, clinical interview with an appropriately trained ADHD clinician, structured ADHD assessment, clinical review, psychologist diagnostic oversight, report preparation, and optional feedback session. Clients may not necessarily meet directly with the psychologist; however, the psychologist is responsible for diagnostic decision-making, final clinical formulation, and report sign off. Cube Clinic does not provide medication prescribing or medication management services. Cube Clinic does not provide crisis care, emergency mental health services, medico-legal assessments, court reports, insurance assessments, compensation assessments, custody assessments, immigration assessments, or ongoing therapy through this booking.

Use of reports

Cube Clinic ADHD assessment reports are prepared for healthcare and personal understanding purposes. Reports must not be represented as medico-legal opinions or relied upon for court, criminal, compensation, insurance, employment dispute, immigration, or forensic purposes unless explicitly stated otherwise in writing by Cube Clinic.

Assessment Process

The assessment uses multiple sources of information including questionnaires, ADHD-specific rating scales, informant information where available, uploaded records where available, clinical interview, structured ADHD interview, and psychologist review. The questionnaires assess attention, concentration, executive functioning, organisation, impulsivity, emotional wellbeing, developmental history, sleep, trauma, substance use, autism traits, and other factors relevant to differential diagnosis and co-occurring conditions. Questionnaires support assessment preparation but do not independently diagnose ADHD. The client confirms that they are the person being assessed and agrees to provide information that is truthful, accurate and complete to the best of their knowledge during booking, questionnaires, uploaded documents and the clinical interview. The client understands that inaccurate, incomplete or misleading information may affect the assessment process, clinical conclusions and resulting report.

Service suitability and telehealth limitations

Cube Clinic provides adult ADHD assessment by telehealth and is not appropriate for people experiencing acute mental health instability, psychosis, mania, or situations requiring urgent or intensive in-person psychiatric care. In some situations, clients may be advised to seek local in-person assessment, crisis support, or ongoing psychiatric care instead. Clients are expected to attend the assessment fit to participate and free from the effects of alcohol or non-prescribed drugs that may impair their judgement, communication or ability to participate meaningfully in the assessment.

Clinical limitations


ADHD symptoms can overlap with stress, burnout, trauma, anxiety, depression, sleep disorders, substance use, autism, learning difficulties, medical conditions, and other mental health or neurodevelopmental conditions. The assessment considers differential diagnosis and co-occurring factors as part of standard clinical practice.

No guaranteed diagnosis

Booking an ADHD assessment does not guarantee an ADHD diagnosis. Assessment outcomes may vary depending on the clinical information available. The assessment may conclude that ADHD is supported, ADHD is supported with co-occurring factors, ADHD traits are present but full criteria are not met, ADHD is not supported, the presentation is mixed or complex, or there is insufficient information to make a definitive diagnostic determination.

Payment

The Adult ADHD Assessment fee is $1,565 + GST ($1800 inc GST). Payment is required at booking. The booking is not secured until payment is completed.

Refunds and released appointments

Payment reserves dedicated clinical assessment time and administrative preparation. If an appointment is released because required questionnaires or assessment requirements were not completed by the deadline, the payment remains attached to the assessment process and the client may rebook once the required information is complete. Refund requests are considered at clinic discretion and in accordance with New Zealand consumer law obligations.

Questionnaires and appointment confirmation

The assessment relies on pre-assessment questionnaires and supporting information. The client must complete the required questionnaires and assessment requirements at least 7 days before the scheduled appointment. If the required questionnaires or assessment requirements are not completed at least 7 days before the appointment, the appointment will be released. The client may rebook once the required questionnaires and assessment requirements are complete. Earlier appointment availability is not guaranteed and wait times may differ from the original booking date.

Informant information

Informant information or alternative supporting information is a standard part of the assessment process where available. The client is asked to nominate someone who can comment on current functioning and, where possible, someone who knew them during childhood. If no informant is available, this is not an automatic barrier to assessment. The clinician will consider what alternative information is available, including school reports, previous assessments, medical records, developmental history, or clinical history.

Uploads

Clients may upload school reports, previous assessments, workplace documentation, medical records, or other supporting documents. Uploads are optional unless specifically requested by the clinician.

Cancellation and rescheduling

The client should provide as much notice as possible if they need to cancel or reschedule an appointment. Clients may cancel or reschedule online through the booking system up to 24 hours before the scheduled appointment time. Clients should not rely on phoning the clinic to cancel or reschedule, as calls may not be monitored immediately. Within 24 hours of the appointment, online cancellation and rescheduling are restricted and any request is at Cube Clinic’s discretion. Cancellation, rescheduling and non-attendance fees are set out below. Cube Clinic recognises that exceptional circumstances may occasionally arise and may, at its discretion, waive or vary these fees where appropriate. Earlier appointments may occasionally be offered to clients who have completed all required assessment steps.

Non-attendance fee

If the client does not attend the scheduled assessment appointment, a non-attendance fee of $350 + GST will be charged to the card used at booking. This fee applies because clinical time has been reserved and cannot usually be reallocated at short notice. If the client books an optional feedback session and does not attend, a non-attendance fee of $180 + GST will be charged to the card used at booking. Non-attendance is separate from short notice cancellation or rescheduling. Where a client cancels or requests to reschedule at short notice, the fee decision follows the cancellation and rescheduling rule above.

Telehealth requirements

The client must attend from a private, quiet space with a stable internet connection. The client must use a device with a camera and microphone. Clients are responsible for ensuring they can access the Google Meet appointment link and should test their device, internet connection, camera and microphone before the appointment. Cube Clinic will provide the Google Meet appointment link in the appointment confirmation and reminder emails. The client must not audio record, video record, photograph, screenshot, screen record, livestream, transcribe, capture, store, distribute, upload, publish, post to social media, or otherwise share any part of the assessment, interview, feedback session or other clinical interaction unless this has been agreed in writing in advance by Cube Clinic. This prohibition includes the use of live transcription services, AI note-taking tools, AI meeting assistants, AI-generated meeting summaries, third-party recording or transcription software, or any similar technology capable of recording, monitoring, transcribing or summarising the session. Unauthorised recording, transcription, uploading, publication or sharing of any part of a clinical interaction may be treated as a serious breach of these Terms of Use. Cube Clinic takes the privacy, confidentiality, clinician identity and integrity of the assessment process seriously. If Cube Clinic becomes aware of any unauthorised recording, transcription or sharing, Cube Clinic may require the material to be removed or deleted, pause services while the matter is reviewed, decline to continue providing services where appropriate, and take any other reasonable steps necessary to protect the privacy, confidentiality, legal rights, safety and integrity of its clients, clinicians and the practice. Clients who wish to have a support person present during the appointment should contact Cube Clinic before the appointment so this can be discussed with and agreed by the clinician. No other person should be present during the appointment unless this has been agreed with the clinician.

Technology issues

If technology fails, Cube Clinic will determine whether the appointment can continue, needs to be paused, or needs to be rescheduled. If the appointment cannot proceed, or is significantly delayed, because of client-side technical difficulties, including internet connection, device, software, failure to access Google Meet, or failure to arrange a suitable private space, this may be managed in accordance with the Cube Clinic cancellation and non-attendance policy, as clinician time has been reserved.

Report release

Reports are released only after psychologist review and approval. The report reflects the psychologist’s final clinical opinion based on the information available at the time of the assessment.

Factual inaccuracies

After receiving the report, the client may notify Cube Clinic of factual inaccuracies. A factual inaccuracy means an incorrect date, name, medication, history detail, or similar factual error. A difference of opinion about the clinical conclusion is not necessarily a factual inaccuracy.

Optional feedback session

The feedback session is optional. It is for discussing the completed report, explaining the findings, answering questions, and discussing recommendations. It is not a repeat assessment or second clinical interview. If important new information comes to light after the report has been completed, Cube Clinic will review what this means. This includes information that was not disclosed, was inaccurately reported, or may affect the assessment outcome. The Clinical Director or responsible psychologist will decide whether any further assessment, report amendment, report withdrawal, referral to another service, or additional appointment is appropriate. Any additional work may be subject to further fees.

Complaints

Cube Clinic is committed to responding to concerns and complaints fairly, respectfully and promptly. Clients are encouraged to raise any concerns with Cube Clinic in the first instance so that they can be reviewed and, where possible, resolved. This does not affect a client’s right to seek independent advice or to make a complaint to an appropriate external authority where applicable. Cube Clinic will respond to complaints in accordance with its professional, legal and ethical obligations.
